Repeat Prescriptions

We would be grateful if could order your routine repeat prescriptions as follows:

– Online: Use the convenient online order form (link below) 
– In Person – download the repeat prescription form, print it out and fill in your prescription list. You can drop this at reception or post it into us. 
– You may also collect a repeat prescription form at reception, fill in your prescription list and drop/post it back into us. 

Terms and Conditions

• To ensure your safety we will need to perform a medical review with you in person once a year, if you on regular medication.
• Any new medications, including antibiotics will require a GP consultation in advance.
• Many medications require monitoring of certain blood tests and vital signs.
• Hospital prescriptions must be forwarded to the surgery so that your medication and medical record can be amended accordingly.
• Chronic Disease Management, CDM, patients are required to have their medication reviewed at their 6 monthly assessment.
• Please allow up to 48 hours turnaround for your prescriptions.
• 17- 35 year olds Free Contraception services require a GP consultation, either in person or by telephone
